' •, 1 JANITORIAL SERVICES AGREEMENT 2 This Agreement is made and entered into this day of 2015, by and 3 between the COUNTY OF FRESNO, a Political Subdivision ofthe State of California, hereinafter 4 referred to as "COUNTY", and "GEIL ENTERPRISES, INC. dba JANITORIAL INC., a Private for 5 Profit Corporation, whose address is 1945 N. Helm Avenue, Suite# 102, hereinafter referred to as 6 "CONTRACTOR". 7 II W I T N E S S E T H: 8 WHEREAS, COUNTY, through its Department of Social Services (DSS) has a need for 9 janitorial services at COUNTY's Huron office located at 36678 S. Lassen, Suite# 1, Huron, CA 10 93234 and Coalinga Regional Center located at 311 Coalinga Plaza, Coalinga, CA 9321 0; and 11 WHEREAS, CONTRACTOR, has the equipment and personnel skilled in the provision of such 12 11 servtces. 1311 NOW, THEREFORE, in consideration oftheir mutual covenants and conditions, the 14 parties hereto agree as follows: 15 16 17 18 19 20 21 22 23 24 25 26 27 28 1. SERVICES A. CONTRACTOR shall provide all services as identified in COUNTY's Request for Quotation (RFQ) No. 485-5372, dated August 13, 2015 and Addendum No. One (1) dated August 25, 2015, collectively hereinafter referred to as COUNTY's Revised RFQ No. 485-5372 and CONTRACTOR's response to said RFQ, dated September 3, 2015, all incorporated herein by reference and made part ofthis Agreement. B. CONTRACTOR shall provide all janitorial services as set forth in Exhibits A, B and C, attached hereto and by this reference incorporated herein. C. In the event of any inconsistencies among the documents described in Paragraphs 1 .A and 1 .B hereinabove, the inconsistency shall be resolved by giving precedence in the following order of priority; 1) to this Agreement, including all Exhibits attached hereto, 2) to the Revised RFQ, 3) to the response to the Revised RFQ. A copy of COUNTY's Revised RFQ No. 485-5372, and CONTRACTOR's response, shall be retained and made available during the term of this Agreement by -1 -COUNTY OF FRESNO Fresno, CA

CONTRACTOR shall ensure that COUNTY's Huron and Coalinga Regional Center facility keys issued to CONTRACTOR for the purpose of carrying out said janitorial services as described hereinabove shall be safeguarded from loss, theft or damage and secured in accordance with the instruction of COUNTY's designee. CONTRACTOR shall identify, and with COUNTY approval, appoint one (1) supervisor to maintain control and responsibility of said facility keys. Violation of the above provision may result in the CONTRACTOR being deemed irresponsible for the purposes of future COUNTY bids and may be grounds for termination of this Agreement at the option of COUNTY. E. All janitorial services, including any additional or emergency services requested by designated COUNTY staff, shall be performed so that any interruption to COUNTY's normal business operations are kept to a minimum. TERM 15 II This Agreement shall become effective on the 1st day of January, 2016 and shall 16 II terminate on the 3151 day ofDecember, 2018. 17 This Agreement shall automatically be extended for two (2) additional twelve (12) -j,b. 18 month periods upon the same terms and conditions herein set forth, unless written notice of non-19 renewal is given by CONTRACTOR or COUNTY or COUNTY's DSS Director, or designee, not later 2 0 than sixty (60) days prior to the close of the current Agreement term. 21 3. TERMINATION 22 A. Non-Allocation of Funds - The terms of this Agreement, and the services to be 23 provided thereunder, are contingent on the approval of funds by the appropriating government agency. 2 4 Should sufficient funds not be allocated, the services provided may be modified, or this Agreement 2 5 terminated at any time by giving CONTRACTOR sixty (60) days advance written notice. 26 B. Breach of Contract -COUNTY may immediately suspend or terminate this 2 7 II Agreement in whole or in part, where in the determination of COUNTY there is: 28 1) An illegal or improper use of funds; -2 -COUNTY OF FRESNO Fresno, CA
1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25 26 27 28 2) A failure to comply with any term of this Agreement; 3) A substantially incorrect or incomplete report submitted to COUNTY; 4) Improperly performed service. In no event shall any payment by COUNTY constitute a waiver by COUNTY of any breach ofthis Agreement or any default which may then exist on the part of CONTRACTOR. Neither shall such payment impair or prejudice any remedy available to COUNTY with respect to the breach or default. COUNTY shall have the right to demand of CONTRACTOR the repayment to COUNTY of any funds disbursed to CONTRACTOR under this Agreement, which in the judgment of COUNTY were not expended in accordance with the terms of this Agreement. CONTRACTOR shall promptly refund any such funds upon demand or, at COUNTY's option, such repayment shall be deducted from future payments owing to CONTRACTOR under this Agreement. C. Without Cause -Under circumstances other than those set forth above, this Agreement may be terminated by CONTRACTOR or COUNTY or COUNTY's DSS Director, or designee, upon the giving of thirty (30) days advance written notice of an intention to terminate the Agreement. 4. COMPENSATION For actual services provided as identified in the terms and conditions of this Agreement, COUNTY agrees to pay CONTRACTOR and CONTRACTOR agrees to receive compensation as set forth in Exhibit D-Budget Summary, attached hereto and by this reference incorporated herein. Payment for services set forth in Exhibits A-C shall be made in twelve equal payments, whenever possible upon certification or other proof satisfactory to COUNTY's DSS that services have actually been performed by CONTRACTOR as specified in this Agreement. Any Additional Services shall be pre-approved by COUNTY's DSS Director or Designee and shall be invoiced and paid for separately upon confirmation of satisfactory work completed. For each twelve (12) month period of this Agreement, in no event shall services performed under this Agreement be in excess of Sixty-Eight Thousand Five Hundred Thirty and ::su''. ,j. No/100 Dollars ($68,530.00). The cumulative total ofthis Agreement shall not be in excess of Three Hundred Forty-Two Thousand Six Hundred Fifty and No/100 Dollars ($342,650.00). 1 that all expenses incidental to CONTRACTOR's performance of services under this Agreement shall 2 be borne by CONTRACTOR. 3 Except as provided below regarding State payment delays, payments by COUNTY shall 4 be in arrears, for services provided during the preceding month, within forty-five (45) days after 5 receipt, verification and approval of CONTRACTOR's invoices by COUNTY's DSS. If 6 CONTRACTOR should fail to comply with any provision of this Agreement, COUNTY shall be 7 relieved of its obligation for further compensation. All final claims and I or any final budget 8 modification requests shall be submitted by CONTRACTOR within sixty (60) days following the final 9 month of service for which payment is claimed. No action shall be taken by COUNTY on claims 10 submitted beyond the sixty (60) day closeout period. Any compensation which is not expended by 11 CONTRACTOR pursuant to the terms and conditions of this Agreement shall automatically revert to 12 II COUNTY. 13 The services provided by CONTRACTOR under this Agreement are funded in whole or 14 in part by the State of California. In the event that funding for these services is delayed by the State 15 Controller, COUNTY may defer payment to CONTRACTOR. The amount ofthe deferred payment 16 shall not exceed the amount of funding delayed by the State Controller to COUNTY. The period of 17 time of the deferral by COUNTY shall not exceed the period of time of the State Controller's delay of 18 payment to COUNTY plus forty-five (45) days. 19 5. INVOICING 2 0 II CONTRACTOR shall invoice COUNTY in arrears by the tenth (1 01h) of each month for 21 actual expenses incurred and services rendered in the previous month to: 2 2 DSSinvoices@co.fresno.ca. us. Invoices shall include all corresponding documentation and identified 2 3 by line item. Supporting documentation shall include but is not limited to receipts or invoices 2 4 received. No reimbursement for services shall be made until invoices and receipts or other supporting 2 5 documents are received, reviewed and approved by COUNTY's DSS. 2 6 At the discretion of COUNTY's DSS Director or designee, if an invoice is incorrect or is 27 otherwise not in proper form or substance, COUNTY's DSS Director or designee shall have the right 2 8 to withhold payment as to only that portion of the invoice that is incorrect or improper after five (5) - 4 -COUNTY OF FRESNO Fresno, CA
1 days prior notice to CONTRACTOR. CONTRACTOR agrees to continue to provide services for a 2 period of ninety (90) days after notification of an incorrect or improper invoice. If after the ninety (90) 3 day period, the invoice(s) is still not corrected to COUNTY's DSS satisfaction, COUNTY's DSS 4 II Director or designee may elect to terminate this Agreement, pursuant to the termination provisions 5 stated in Paragraph Three (3) of this Agreement. In addition, for invoices received ninety (90) days 6 after the expiration of each term of this Agreement or termination of this Agreement, at the discretion 7 of COUNTY's DSS Director or designee, COUNTY's DSS shall have the right to deny payment of 8 II any additional invoices received. 9 6. INDEPENDENT CONTRACTOR 10 In performance of the work, duties, and obligations assumed by CONTRACTOR under 11 this Agreement, it is mutually understood and agreed that CONTRACTOR, including any and all of 12 CONTRACTOR's officers, agents, and employees will at all times be acting and performing as an 13 independent contractor, and shall act in an independent capacity and not as an officer, agent, servant, 14 employee, joint venturer, partner, or associate of COUNTY. Furthermore, COUNTY shall have no 15 right to control or supervise or direct the manner or method by which CONTRACTOR shall perform 16 its work and function. However, COUNTY shall retain the right to administer this Agreement so as to 17 verify that CONTRACTOR is performing its obligations in accordance with the terms and conditions 18 thereof. CONTRACTOR and COUNTY shall comply with all applicable provisions of law and the 19 rules and regulations, if any, of governmental authorities having jurisdiction over matters which are 2 0 directly or indirectly the subject of this Agreement. 21 Because of its status as an independent contractor, CONTRACTOR shall have 2 2 absolutely no right to employment rights and benefits available to COUNTY employees. 2 3 CONTRACTOR shall be solely liable and responsible for providing to, or on behalf of, its employees 2 4 all legally-required employee benefits. In addition, CONTRACTOR shall be solely responsible and 2 5 save COUNTY harmless from all matters relating to payment of CONTRACTOR's employees, 2 6 including compliance with Social Security, withholding, and all other regulations governing such 2 7 matters. It is acknowledged that during the term of this Agreement, CONTRACTOR may be 2 8 providing services to others unrelated to COUNTY or to this Agreement. -5 -COUNTY OF FRESNO Fresno, CA
1 2 7. MODIFICATION A. Any matters of this Agreement may be modified from time to time by the written 3 II consent of all the parties without, in any way, affecting the remainder. 4 B. CONTRACTOR hereby agrees that changes to the compensation under this 5 Agreement may be necessitated by a reduction in funding from State and/or Federal sources. 6 COUNTY's DSS Director or designee may modifY the maximum compensation depending on State 7 and Federal funding availability, as stated in Section Four (4) in this Agreement. CONTRACTOR 8 further understands that this Agreement is subject to any restrictions, limitations or enactments of all 9 legislative bodies which affect the provisions, term or funding of this Agreement in any manner. 10 8. NON-ASSIGNMENT 11 II Neither party shall assign, transfer or subcontract this Agreement nor their rights or 12 duties under this Agreement without the prior written consent of the other party. 13 9. HOLD-HARMLESS 14 CONTRACTOR agrees to indemnify, save, hold harmless, and at COUNTY's request, 15 defend COUNTY, its officers, agents and employees from any and all costs and expenses, including 16 attorney fees and court costs, damages, liabilities, claims and losses occurring or resulting to 17 COUNTY in connection with the performance, or failure to perform, by CONTRACTOR, its officers, 18 agents or employees under this Agreement, and from any and all costs and expenses, including 19 attorney fees and court costs, damages, liabilities, claims and losses occurring or resulting to any 2 0 person, firm or corporation who may be injured or damaged by the performance, or failure to perform, 21 of CONTRACTOR, its officers, agents or employees under this Agreement. In addition, 22 CONTRACTOR agrees to indemnifY COUNTY for Federal, State of California and/or local audit 2 3 exceptions resulting from noncompliance herein on the part of the CONTRACTOR. 24 10. INSURANCE 2 5 II Without limiting COUNTY's right to obtain indemnification from CONTRACTOR or 2 6 II any third parties, CONTRACTOR, at its sole expense, shall maintain in full force and effect the 2 7 11 following insurance policies throughout the term of this Agreement: 28 A. Commercial General Liability Insurance with limits of not less than One Million Dollars ($1,000,000) per occurrence and an annual aggregate ofTwo Million Dollars ($2,000,000). This policy shall be issued on a per occurrence basis. COUNTY may require specific coverage including completed operations, product liability, contractual liability, Explosion, Collapse, and Underground (XCU), fire legal liability or any other liability insurance deemed necessary because of the nature of the Agreement. Automobile Liability Comprehensive Automobile Liability Insurance with limits for bodily injury of not less than Two Hundred Fifty Thousand Dollars ($250,000) per person, Five Hundred Thousand Dollars ($500,000) per accident and for property damages of not less than Fifty Thousand Dollars ($50,000), or such coverage with a combined single limit of Five Hundred Thousand Dollars ($500,000). Coverage should include owned and non-owned vehicles used in connection with this Agreement. Professional Liability ,Jff< J,~. If CONTRACTOR employs licensed professional staff (e.g. Ph.D., R.N., L.C.S.W., M.F.C.T.) in providing services, Professional Liability Insurance with limits of not less than One Million Dollars ($1 ,000,000) per occurrence, Three Million Dollars ($3,000,000) annual aggregate. Worker's Compensation A policy of Worker's Compensation Insurance as may be required by the California Labor Code. CONTRACTOR shall obtain endorsements to the Commercial General Liability insurance naming the County of Fresno, its officers, agents, and employees, individually and collectively, as additional insured, but only insofar as the operations under this Agreement are concerned. Such coverage for additional insured shall apply as primary insurance and any other insurance, or self-insurance, maintained by COUNTY, its officers, agents and employees shall be excess only and not contributing with insurance provided under CONTRACTOR's policies herein. This insurance shall not be cancelled or changed without a minimum of thirty (30) days advance written notice given to COUNTY. Within thirty (30) days from the date CONTRACTOR signs this Agreement, CONTRACTOR shall provide certificates of insurance and endorsements as stated above for all of -7 -COUNTY 011 FRESNO Fresno, CA
1 the foregoing policies, as required herein, to the County ofFresno, DSS, P.O. BOX 1912, Fresno, 2 California, 93718-1912, Attention: Kent Luckin, Senior Staff Analyst, stating that such insurance 3 coverages have been obtained and are in full force; that the County of Fresno, its officers, agents and 4 employees will not be responsible for any premiums on the policies; that such Commercial General 5 Liability insurance names the County of Fresno, its officers, agents and employees, individually and 6 collectively, as additional insured, but only insofar as the operations under this Agreement are 7 concerned; that such coverage for additional insured shall apply as primary insurance and any other 8 insurance, or self-insurance, maintained by COUNTY, its officers, agents and employees, shall be 9 excess only and not contributing with insurance provided under CONTRACTOR's policies herein; 10 and that this insurance shall not be cancelled or changed without a minimum of thirty (30) days 11 advance, written notice given to COUNTY. 12 In the event CONTRACTOR fails to keep in effect at all times insurance coverage as 13 herein provided, COUNTY may, in addition to other remedies it may have, suspend or terminate this 14 Agreement upon the occurrence of such event. 15 All policies shall be with admitted insurers licensed to do business in the State of 16 California. Insurance purchased shall be from companies possessing a current A.M. Best, Inc. rating 17 II of A FSC VII or better. 18 11. CONFIDENTIALITY 19 All services performed by CONTRACTOR under this Agreement shall be in strict 2 0 conformance with all applicable Federal, State of California and/or local laws and regulations relating 21 to confidentiality. 22 12. NON-DISCRIMINATION 23 During the performance ofthis Agreement CONTRACTOR shall not unlawfully 2 4 discriminate against any employee or applicant for employment, or recipient of services, because of 2 5 ethnic group identification, gender, gender identity, gender expression, sexual orientation, color, 2 6 physical disability, mental disability, medical condition, national origin, race, ancestry, marital status, 2 7 religion, or religious creed, pursuant to all applicable State of California and Federal statutes and 2 8 regulations. -8 -COUNTY 01' FRESNO l'resno, CA

CONFLICT OF INTEREST No officer, agent, or employee of COUNTY who exercises any function or responsibility for planning and carrying out the services provided under this Agreement shall have any direct or indirect personal financial interest in this Agreement. No officer, agent, or employee of COUNTY who exercises any function or responsibility for planning and carrying out the services provided under this Agreement shall have any direct or indirect personal financial interest in this Agreement. In addition, no employee of COUNTY shall be employed by CONTRACTOR to fulfill any contractual obligations with COUNTY. CONTRACTOR shall also comply with all Federal, State of California, and local conflict of interest laws, statutes, and regulations, which shall be applicable to all parties and beneficiaries under this Agreement and any officer, agent, or employee of COUNTY. 14. CHARITABLE CHOICE CONTRACTOR may not discriminate in its program delivery against a client or potential client on the basis of religion or religious belief, a refusal to hold a religious belief, or a refusal to actively participate in a religious practice. Any specifically religious activity or service made available to individuals by CONTRACTOR must be voluntary as well as separate in time and location from County funded activities and services. CONTRACTOR shall inform COUNTY as to whether it is faith-based. If CONTRACTOR identifies as faith-based it must submit to DSS a copy of its policy on referring individuals to an alternate treatment provider, and include a copy of this policy in its client admission forms. The policy must inform individuals that they may be referred to an alternative provider if they object to the religious nature of the program, and include a notice to DSS. Adherence to this policy will be monitored during annual site reviews, and a review of client files. If CONTRACTOR identifies as faith-based, by July 1 of each year CONTRACTOR will be required to report to DSS the number of individuals who requested referrals to alternate providers based on religious objection. 15. PROHIBITION ON PUBLICITY None of the funds, materials, property or services provided directly or indirectly under this Agreement shall be used for CONTRACTOR's advertising, fundraising, or publicity (i.e., purchasing of tickets/tables, silent auction donations, etc.) for the purpose of self-promotion. -9 -COUNTY OF FRESNO Fresno, CA
. 1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25 26 27 28 Notwithstanding the above, publicity of the services described in Paragraph One (1) of this Agreement shall be allowed as necessary to raise public awareness about the availability of such specific services when approved in advance by the Director or designee and at an approved cost for such items as written/printed materials, the use of media (i.e., radio, television, newspapers) and any other related expense( s). 16. DISCLOSURE OF SELF -DEALING TRANSACTIONS This provision is only applicable if CONTRACTOR is operating as a corporation (a for-profit or non-profit corporation) or if during the term of this Agreement, CONTRACTOR changes its status to operate as a corporation. Members of CONTRACTOR's Board of Directors shall disclose any self-dealing transactions that they are a party to while CONTRACTOR is providing goods or performing services under this Agreement. A self-dealing transaction shall mean a transaction to which CONTRACTOR is a party and in which one or more of its directors has a material financial interest. Members of the Board of Directors shall disclose any self-dealing transactions that they are a party to by completing and signing a Self-Dealing Transaction Disclosure Form, attached hereto as Exhibit E and by this reference incorporated herein, and submitting it to COUNTY prior to commencing with the self-dealing transaction or immediately thereafter. 17. AUDITS AND INSPECTIONS CONTRACTOR shall at any time during business hours, and as often as COUNTY may deem necessary, make available to COUNTY for examination all of its records and data with respect to the matters covered by this Agreement. CONTRACTOR shall, upon request by COUNTY, permit COUNTY to audit and inspect all such records and data necessary to ensure CONTRACTOR's compliance with the terms of this Agreement. If this Agreement exceeds Ten Thousand and No/100 Dollars ($10,000.00), CONTRACTOR shall be subject to the examination and audit of the State of California Auditor General for a period ofthree (3) years after final payment under contract (California Government Code section 8546. 7). 18. CHANGE OF LEADERSHIP I MANAGEMENT 8 Any and all notices between COUNTY and CONTRACTOR provided for or permitted 9 under this Agreement or by law, shall be in writing and shall be deemed duly served when personally 10 delivered to one of the parties, or in lieu of such personal service, when deposited in the United States 11 Mail, postage prepaid, addressed to such party. 12 In the event of any change in the status of CONTRACTOR'S leadership or 13 management, CONTRACTOR shall provide written notice to COUNTY within thirty (30) days from 14 the date of change. Such notification shall include any new leader or manager's name, address and 15 qualifications. "Leadership or management" shall include any employee, member, or owner of 16 CONTRACTOR who either a) directs individuals providing services pursuant to this Agreement, b) 1 7 exercises control over the manner in which services are provided, or c) has authority over 18 II CONTRACTOR's finances. 19 20. GOVERNING LAW 2 0 The parties agree, that for the purposes of venue, performance under this Agreement is 21 to be in Fresno County, California. 2 2 The rights and obligations of the parties and all interpretation and performance of this 2 3 Agreement shall be governed in all respects by the laws of the State of California. 24 21. ENTIRE AGREEMENT 2 5 This Agreement, including all Exhibits, COUNTY's Revised RFQ No. 485-5372 and 2 6 CONTRACTOR's response thereto, constitutes the entire agreement between CONTRACTOR and 2 7 COUNTY with respect to the subject matter hereof and supersedes all previous agreement 2 8 negotiations, proposals, commitments, writings, advertisements, publications and understandings of -11 -COUNTY OF FRESNO Fresno, CA

r Janitorial Service Requirements General Specifications: Exhibit A Page 1 of 3 1 . The general day-to-day housekeeping activities are performed in accordance with commercial building housekeeping standards. 2. A schedule of housekeeping activities is also attached to this Agreement, and identified as Exhibit "B" and "C". These exhibits are to be used as a daily check-off list by the vendor. 3. There shall be sufficient housekeeping cleaning supplies and equipment provided to perform duties on a daily basis. 4. Housekeeping cleaning supplies and equipment shall be stored in designated housekeeping closets or areas. 5. A detergent germicide shall be used for all cleaning and dusting purposes. 6. Mop heads shall be removable and changed regularly for sanitary purposes. 7. There shall be sufficient housekeeping personnel to maintain the interior of the buildings in a safe, clean, orderly, attractive manner and free from offensive odors. 8. Unexpected or unscheduled needs for housekeeping services shall be categorized into three priority levels. A. The highest priority services shall be those required for safety and sanitation. Any unsafe or unsanitary condition shall be corrected as soon as possible (always within twenty-four (24) hours). B. The second priority level shall be services required for the general functioning of the facility, but for which no hazard is present. These shall be corrected as soon as possible, with written timelines for repairs provided to the designated Department of Social Services Facility Resources Analyst. C. All other situations shall be handled on a scheduled basis. DAILY CLEANING: 1. Vacuum all carpets thoroughly: A. Under and around furniture. B. Corners, behind doors and along the baseboards. C. Spot clean spills, stains and remove gum. 2. Empty all wastebaskets and waste containers and replace liners as needed. 3. Damp wipe all counters, modular furniture work surfaces, and desktops, provided they are free of work materials. 082 I aadx -I -
4. Dust (with treated mop), spot clean and damp mop hard surface floors. Clean elevator (Coalinga location only): A. Stainless steel areas are to be cleaned with a metal polish. B. Wood finish areas are to be cleaned with furniture polish. C. Damp wipe walls. D. Floors/carpets are to be vacuumed. E. Elevator tracks are to be cleaned and vacuumed. 5. Clean up all spillage. 6. Clean all entry/exit doors and door glass, inside and outside. 7. Dust with treated mop and damp mop all outside and inside entry floors. 8. Restrooms: Exhibit A Page 2 of 3 A. Floors are to be mopped with a disinfectant approved by Fresno County. B. Fixtures on the sinks, toilets, urinals, and the mirrors are to be cleaned with a disinfectant approved by Fresno County. C. Toilets and urinals, plus the surrounding wall areas, are to be cleaned with a disinfectant approved by Fresno County. D. Sinks are to be cleaned with a powder cleanser containing bleach, if permitted. E. Tile surrounding sinks are to be cleaned using a disinfectant approved by Fresno County. F. Empty wastebasket containers. G. Refill toilet paper, paper towel, soap dispensers, sanitary products, air freshener dispensers, and seat covers. H. Other cleaning agents to remove resistant soil, polish metal, or preserve floors should be used as needed, after any required treatment with disinfectant cleaner. Cleaning agents to be approved by Fresno County. I. Clean all toilet paper, paper towel and soap dispensers. 9. Spot clean windows, walls, doors, door jams and area around light switches. 10. Clean with sanitizing disinfectant doorknobs and stairwell handrails. 11. Remove empty boxes, cardboard and miscellaneous material and place in appropriate containers. 12. Damp wipe chairs and/or benches in lobby/waiting room. 13. Remove staples and paper clips from carpet. 14. Clean sinks and counters in break rooms. 0821 aadx -2 -
15. Remove cobwebs. 16. Clean door thresholds. 17. Replace all inoperative light bulbs/fluorescent tubes. 18. Wash fingerprints from walls. 19. Remove graffiti from wall areas. WEEKLY CLEANING: 1. if EXHIBITE SELF-DEAUNG TRANSACTION DISCLOSURE FORM In order to conduct business with the County of Fresno (hereinafter referred to as "County"), members of a contractor's board of directors (hereinafter referred to as "County Contractor"), must disclose any self-dealing transactions that they are a party to while providing goods, performing services, or both for the County. A self-dealing transaction is defined below: "A self-dealing transaction means a transaction to which the corporation is a party and in which one or more of its directors has a material financial interest" The definition above will be utilized for purposes of completing this disclosure form. INSTRUCTIONS (1) Enter board member's name, job title (if applicable), and date this disclosure is being made. (2) Enter the board member's company/agency name and address. (3) Describe in detail the nature of the self-dealing transaction that is being disclosed to the County. At a minimum, include a description of the following: a.
