3 THIS AMENDMENT IV TO AGREEMENT(hereinafter "Amendment IV") is made
4 and entered into this 22"d day of August, 2017, by and between COUNTY OF FRESNO, a
5
Political Subdivision of the State of California, Fresno, California (hereinafter
6
7 "COUNTY"), and Risico, Inc., whose address is 2425 W Shaw Ave, Fresno, CA 93711
8 (hereinafter"CONTRACTOR").

10 WITNESSETH:

12 WHEREAS, COUNTY and CONTRACTOR entered into Agreement number 12-253,
13 effective October 1, 2012, which the parties thereafter amended by executing Amendment 1
14 effective April 1, 2014, Amendment II effective July 29, 2014 and Amendment III effective
15
October 27, 2015 (all collectively referred to as the "Agreement), pursuant to which
16
17 CONTRACTOR agreed to provide workers' compensation claims administration and selected
18 ancillary services to COUNTY; and
19
20 WHEREAS, COUNTY and CONTRACTOR now desire to amend the Agreement in order to
21 enter into a ninety (90) day extension of the current contract that is set to expire on September
22 30, 2017.

NOW, THEREFORE for good and valuable consideration, the receipt and adequacy of
24
25 which is hereby acknowledged, COUNTY and CONTRACTOR agree as follows:

1) The Term of the Agreement, set forth in Section Two (2) of the Agreement,
27
28 shall be extended an additional three months, terminating December 31, 2017.

1 2) During the period October 1, 2017 through December 31, 2017, COUNTY shall pay
2 CONTRACTOR for services rendered as set forth in the Agreement. During the period
3
October 1, 2017 through December 31, 2017, in no event shall Claims Administration
4
Services performed be in excess of Two Hundred Fifty Five Thousand Dollars ($255,000
5
6 and in no event shall Medical Management fees for ancillary services be in excess of
7 Sixty-Nine Thousand Nine Hundred Ninety Nine Dollars and Ninety Nine Cents
8 ($69,999.99).

COUNTY and CONTRACTOR agree that this Amendment IV is sufficient to
10
11 amend the Agreement and, that upon execution of this Amendment IV, the Agreement,
12
13 Amendment I, Amendment II, Amendment III and this Amendment IV together shall be
14
considered the Agreement.

16 The Agreement, as hereby amended, is ratified and continued. All provisions,
17
18 terms, covenants, conditions and promises contained in the Agreement and not amended
19
20 herein shall remain in full force and effect.
21 INWITNESS WHEREOF, the parties hereto have executed this Amendment
22
IV to Agreement No. 12-253 as of the day and year first hereinabove
23
written.
