COUNTY OF FRESNO CONSTRUCTION MANAGEMENT CONTRACT CHANGE ORDER AUBERRY ROAD/COPPER AVENUE CHANGE ORDER NO. 05 TRAFFIC SIGNAL INSTALLATION JANUARY 10, 2023 FEDERAL PROJECT NUMBER: HSIPL-5942(241) PAGE 2 OF 5 CONTRACT NO. 19-08-C PART 11: INSTALL STEEL BOLLARDS AT THE EQUIPMENT PAD—EXTRA WORK This change was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, install a total of three (3)4-inch steel bollards on the traffic-side of the equipment pad at the north-west corner, as shown on the drawing. Install bollards in a 12-inch diameter and 30-inch-deep concrete footing, with a 6-inch concrete cover at the bottom of the bollards. Utilize minor concrete conforming to the requirements of Section 90-2, "Minor Concrete,"of the Standard Specifications. Compensation for this work will be made at force account on time and materials basis, as per Section 9-1.03, "Payment Scope,"of the Special Provisions. Payment for this work was calculated according to Section 9-1.04, "Force Account," of the Standard Specifications, resulted in $4,028.51. This extra work was performed during the contract suspension,therefore, granting additional contract time was not warranted. In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, install a total of eight traffic sensor loops (five Type E and three Type D)for the southbound Auberry Road and eastbound Copper Avenue per the attached revised striping plan. Perform all work according to Section 23,"Traffic Signals and Street Lighting,"of the City of Fresno Standards (included in the Project Details, in Project Specifications). Payment for this work will be made as extra work at an agreed lump sum price of$9,889.33 according to Section 9-1.03, "Payment Scope,"of the Special Provisions.This price includes compensation of all labor, materials, equipment, overhead costs, markups, and incidentals for performing the work. This extra work was performed during the contract suspension, therefore, granting additional contract time was not warranted. Perform all work according to Section 23, "Traffic Signals and Street Lighting,"of the City of Fresno Standards(included in the Project Details, in Project Specifications). Compensation for this work will be made at force account on time and materials basis,as per Section 9-1.03, "Payment Scope,"of the Special Provisions. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, extend the Midwest Guardrail System by 25 feet along southside of Copper Avenue between sta. 19+70.86 and 19+95.86. Perform all work according to Section 83-2.02, "Midwest Guardrail Systems,"of the Standard Specifications. Payment for this work will be made as extra work at an agreed lump sum price of$6,431.65 according to Section 9-1.03, 'Payment Scope,"of the Special Provisions.This price includes compensation of all labor, materials, equipment, overhead costs, markups, and incidentals for performing the work. This extra work was performed during the contract suspension, therefore, granting additional contract time was not warranted. TOTAL COST PART VI: A $3,449.60 INCREASE TOTAL TIME EXTENSION PART VI: ZERO (0)-WORKING DAYS PART VII: INSTALL ADDITIONAL TEMPORARY TRAFFIC CONTROL DEVICES FOR THE SIGNAL TURN- ON—EXTRA WORK This change was requested by the engineer. In accordance with Section 4-1.05, "Changes and Extra Work,"of the Standard Specifications, furnish and install a total of three (3) portable changeable message signs (PCMS), two(2)"STOP AHEAD"signs,and two (2) "STOP"signs at the locations determined by the Resident Engineer_The temporary roadside signs are to be installed on Type III Barricades. These signs and the PCMSs are to be displayed for two weeks and removed when the newly installed traffic signal is fully functional. Perform all work according to Section 12, "Temporary Traffic Control,"of the Project Special Provisions and Standard Specifications. Payment for this work will be made as extra work at an agreed lump sum price of$4,553.12 according to Section 9-1.03, "Payment Scope,"of the Special Provisions.This price includes compensation of all labor, materials, equipment, overhead costs, markups, and incidentals for performing the work. This extra work was performed during the contract suspension, therefore, granting additional contract time was not warranted. WHY: The item quantities in the Bid Book are estimates only. This reflects the actual quantities used for construction. Bid Item No. 23 — Remove Roadside Sign increased by 200 percent, reflects the actual used quantity in the field. Payment for the increased quantity in excess of 125 percent of the estimated quantity resulted in an amount of$30. Since the payment for the quantity in excess of 125 percent resulted in less than $5,000, no adjustment to the unit price was made as per Section 9-1.06, "Changed Quantity Payment Adjustments," of the Standard Specifications; and the increased quantity was paid at the unit price, resulting in an increase of$120. Bid Item No. 42 — Paint Pavement Marking (Retroreflective) Bike Loop Symbol (2-Coat) decreased by 37.75 percent, reflects the actual used quantity in the field. The contractor did not request a pay adjustment for the quantity decreased more than 25 percent, as referenced in Section 9-1.06C "Decreases of More Than 25 percent," of the Standard Specifications. This bid item was decreased by 37.75 percent, resulting in a $33.00 decrease. All other changes in the bid item quantities are minor and within the normal deviations for this type of construction. HOW: Item quantities were increased or decreased at unit prices per Section 9-1.06, "Changed LETTER OF TRANSMITTAL - C.C.O. NO. 5 JANUARY 10, 2023 Quantity Payment Adjustments," of the Standard Specifications, resulting in an increase of $457.00. COST: A $457.00 Increase PART II - DESCRIPTION: Install steel bollards at the equipment pad —extra work. WHY: The PG&E requested installing three (3) 4-inch steel bollards on the traffic-side of the signal equipment pad. This work was not included in the project; therefore, it was necessary to perform this work as extra work. HOW: The contractor was directed to perform this extra work according to Section 4-1.05, "Changes and Extra Work," of the Standard Specifications. Compensation for this work will be made at force account on time and materials basis, per Section 9-1.03, "Payment Scope," of the Special Provisions. The County staff kept track of labor, equipment, and materials used for this work and calculated compensation according to Section 9-1.04, "Force Account," of the Standard Specifications. COST: A $4,028.51 Increase PART III - DESCRIPTION: Install traffic sensor loops per the revised striping plan — extra work WHY: The original striping plan was revised to accommodate truck turns through the intersection. The contractor had already installed the traffic sensor loops and the temporary striping per the original striping plan. The revised striping plan required re-installing a total of eight (8) traffic sensor loops at the new locations on southbound Auberry Road and eastbound Copper Avenue; this was considered extra work. HOW: The contractor was directed to perform this extra work according to Section 4-1.05, "Changes and Extra Work," of the Standard Specifications. Compensation for this work will be made at an agreed lump sum price of$9,889.33 according to Section 9-1.03, "Payment Scope," of the Special Provisions. COST: A $9,889.33 Increase PART IV - DESCRIPTION: LETTER OF TRANSMITTAL - C.C.O. NO. 5 JANUARY 10, 2023 Splice new traffic sensor loops —extra work. WHY: The new traffic senor loops installed as extra work, included in Part III of this contract change order, required splicing into the traffic signal components, this work was also considered as extra work. HOW: The contractor was directed to perform this extra work according to Section 4-1.05, "Changes and Extra Work," of the Standard Specifications. Compensation for this work will be made at force account on time and materials basis, per Section 9-1.03, "Payment Scope," of the Special Provisions. The County staff kept track of labor, equipment, and materials used for this work and calculated compensation according to Section 9-1.04, "Force Account," of the Standard Specifications. COST: A $3,449.60 Increase PART VII- DESCRIPTION: Install additional temporary traffic control devices for the signal turn-on —extra work. WHY: This project changed the intersection control from a one-way stop operation to a fully signalized operation. In order to safely transition from a one-way stop to a full signalized control, the County staff directed the contractor to install a total of three (3) portable changeable message signs (PCMS), two (2) "STOP AHEAD" signs, and two (2) "STOP" signs for two weeks while the signal was put on flash-red mode. Placement of these temporary traffic control devices was considered as extra work. HOW: The contractor was directed to perform this extra work according to Section 4-1.05, "Changes and Extra Work," of the Standard Specifications. Compensation for this work will be made at an agreed lump sum price of$4,553.12 according to Section 9-1.03, "Payment Scope," of the Special Provisions.
