Make the following changes to the plans and specifications or do work not included in the plans and
specifications for this contract. Because of the change(s), ZERO (0) -WORKING DAYS will be added to the
contract time. The completion date was September 15, 2022.
PART I: ADJUST UNIT PRICE FOR THE QUANTITY OF BID ITEM NO. 46, MIDWEST GUARDRAIL
SYSTEM (WOOD POST), THAT DECREASED MORE THAN 25 PERCENT
This unit price adjustment was requested by the contractor.
Per Section 9-1.06C, "Decreases of More Than 25 Percent," of the Standard Specifications, adjust unit
price of the decreased quantity of Bid Item No. 46, Midwest Guardrail System (Wood Post). The original bid
quantity was 208 linear feet of Midwest Guardrail System, and only a total of 24 linear feet of Midwest
Guardrail System was placed, decreasing the bid quantity by 88.46 percent. The contractor requested a unit
price adjustment in accordance with the above referenced section, with an additional amount of$2,782.08. The
requested unit price adjustment does not exceed the value of 75 percent of the original bid quantity. Total
compensation for the unit price adjustment resulted in an increase of$2,782.08.
This unit price adjustment did not impact the critical path for the project, therefore, granting additional contract
time was not warranted.

PART II: MODIFY DRAINAGE, AND ADD STRIPING AND ADDITIONAL ROADSIDE SIGNAGE — EXTRA
WORK
This change was proposed by the engineer.
In accordance with Section 4-1.05, "Changes and Extra Work," of the Standard Specifications, modify
drainage and install striping and additional roadside signage as per the revised plan sheets (attached).
The work includes but is not limited to extending the 36" drainage riser, placing additional Class I rock in
drainage ditches and around drainage riser, installing thermoplastic centerline and edge line stripes on
Ennis Road, and installing an object marker and arrow signage within the project site. Provide all labor,
materials, equipment, and incidentals for completing the change order work. Perform the work according
to the applicable sections of the Standard Specifications and Special Provisions. Payment for this work will
be made as extra work at an Agreed Price according to Section 9-1.03, "Payment Scope," of the Standard
Specifications. Total compensation for this work resulted in a $19,811.26 increase.

This work was performed after completion of the Second Order of Work (Physical Construction), therefore,
granting additional contract time was not warranted.

PART I - DESCRIPTION:
Adjust unit price for the quantity of Bid Item No. 46 Midwest Guardrail System (Wood Post), that
decreased more than 25 percent.
WHY:
Bid Item No. 46, Midwest Guardrail System (Wood Post) had a contract quantity of 208 linear
feet of guardrail, but only 24 linear feet of Midwest Guardrail System was installed. In
accordance with Section 9-1.06C, "Decreases of More Than 25 Percent," of the Standard
Specifications, the contractor requested for unit bid price adjustment. The requested amount
was negotiated to an agreed price that did not exceed 75 percent of the value of the original
bid quantity (documents provided). Total compensation for the unit price adjustment resulted
in an increase of$2,782.08.
HOW:
Utilize methods established in Section 9-1.06C, "Decreases of More Than 25 Percent," of the
Standard Specifications.
COST:
A $2,782.08 Increase

PART II - DESCRIPTION:
Modify drainage, and add striping and additional roadside signage —extra work.
WHY:
The Department elected to make post construction design changes to the drainage ditch and to
add roadway striping and additional roadside signage throughout the project site for additional
public safety. This work was not included in the contract; therefore, it was necessary to perform
this work as extra work.
HOW:
The contractor was directed to perform post-construction design revisions on previously
installed drainage, install additional signage, and thermoplastic striping according to Section 4-
1.05, "Changes and Extra Work," of the Standard Specifications. The contractor will be
compensated for the labor, equipment, material, and any other related costs associated with the
design revisions. Compensation for this work will be made at an agreed lump sum price of
$19,811.26, in accordance with Section 9-1.03, "Payment Scope," of the Standard
Specifications.
COST:
A $19,811.26 Increase
