RE: One-Year Extension of Agreement No. 18-599
Fresno County Agreement No. 18-599 which was approved by the Board of Supervisors on
October 23, 2018, allows for the option of two one-year extensions upon written approval of
both parties, and authorizes the Department of Behavioral Health Director, or designee, to
execute such written approval on behalf of the County.
The current term of the agreement is scheduled to expire on June 30, 2021, therefore, DBH
would like to extend the Agreement term from July 1, 2021 through June 30, 2022. If you, as the
signing authority for The Fresno Center, would also like the Agreement to be extended as
stated, please sign and date on the line below then email this letter to
eoonzalez(a)fresnocountyca.gov and return this letter to the following address as soon as
possible:
