Re: One-Year Extension of Agreement No. A-19-122 — Implementation of Food Service
Guidelines and Interventions Supportive of Breastfeeding
Dear Mr. Ramirez and Mr. Blalock:
The County's Agreement No. A-19-122, which was approved by the Board of
Supervisors on March 12, 2019, allowed for the option of two one-year extensions upon
written approval of both parties no later than thirty (30) days prior to the first day of the
next twelve-month extension period (October 1, 2021) and authorized the Department
of Public Health Director or his or her designee to execute such written approval on
behalf of the County based on Contractor's satisfactory performance.
I have reviewed the performance of California Health Collaborative, in providing the
contracted services and find it continues to be satisfactory. Therefore, I would like to
extend the agreement from October 1, 2021 through September 30, 2022. If you, as
the signing authority for California Health Collaborative would also like the agreement to
be extended as stated, please sign and date on the line below and return this letter by
September 20, 2021 to:
