HOLT DIRECTOR OF BEHAVIORAL HEALTH 5 PUBLIC GUARDIAN April 28, 2022 Marilyn Bamford Uplift Family Services 251 Llewellyn Avenue Campbell, CA 95008 RE: One-Year Extension of Agreement No. A-19-277-1 Dear Ms. Bamford: Agreement No. 19-277-1, which was approved by the Board of Supervisors on June 18, 2019, and amended on April 13, 2021, allows for the option of two one-year extensions upon written approval of both parties and authorized the Director to execute such written approval on behalf of the County. Department staff have reviewed the performance of Uplift Family Services' Hope program in providing the contracted services and find it continues to be satisfactory. On March 1, 2022, Uplift Family Services has merged with Pacific Clinics and is now conducting services as Pacific Clinics dba Uplift Family Services. Starting July 1, 2021, documents will be updated, and the agency will be fully known as Pacific Clinics. Therefore, the extension for Fiscal Year 2022-23 will not be extended to Uplift Family Services. However, the department will await new contract documents from Pacific Clinics for addition to Master Agreement No. 19-277-1 for Fiscal Year 2022-23. If you, as the signing authority for Pacific Clinics, agree with this understanding, please sign and date on the line below and return this letter by e-mailing a copy of the signed letter to Emma Mejia, Senior Staff Analyst at emmejia(aDfresnocountyca.gov.
