RE: One-Year Extension of Agreement No. A-19-200
Dear Dana Massey:
County of Fresno Agreement No. A-19-200, which was approved by the Board of Supervisors on May
7, 2019 allowed for the option of two (2) twelve-month extensions upon written approval of both
parties no later than thirty days prior to the first day of the next twelve-month extension period, which
would begin July, 1, 2022, and authorized the Director or designee of the Department of Behavioral
Health to execute such written approval on behalf of the County.
The Department finds that Jackson & Coker Locum Tenens, Inc. continues to provide satisfactory
services, and would like to extend the agreement from July 1, 2022 through June 30, 2023. If you
agree to extend Agreement No. A-19-200 as stated, please sign and date below, and return this
letter by June 1, 2022 to:
