Re: Notice of Termination, Contract Number: A-21-537
Dear Mr. Wilson:
The letter serves as written notice from the County of Fresno to terminate the Service Agreement (A-
21-537) between the County of Fresno, Department of Public Health, and HR Support, Inc.
Pursuant to Paragraph 413 of this Agreement, please accept this letter as advance written notice of our
intention to terminate the above-referenced Agreement. The effective date of termination shall be May
20, 2022.
The Department of Public Health has enjoyed working in collaboration with HR Support providing
COVID vaccination and testing services; however, the agreement is being terminated as a result of a
significant decrease in demand for these services.
