RE: One-Year Extension of Agreement No. 18-666-1
Dear Melissa Evans:
County of Fresno Agreement No. 18-666-1, which was approved by the Board of Supervisors on
December 4, 2018 allows for the option of two (2) one-year extensions upon written approval of
both parties no later than thirty days prior to the first day of the next twelve-month extension
period, July, 1, 2022, and authorized the Director of the Department of Behavioral Health to
execute such written approval on behalf of the County.
I have reviewed the performance of Supplemental Healthcare Services, Inc in providing
temporary staffing contracted services and find it continues to be satisfactory. Therefore, I would
like to extend the agreement from July 1, 2022 through June 30, 2023. If you, as the signing
authority for Supplemental Healthcare Services, Inc, would also like the agreement to be
extended as stated, please sign and date on the line below and return this letter by May 31, 2022
