Re: Notice of Termination of Agreement A-21-194
Dear Mr. Nkanch:
The County of Fresno, through its Department of Public Health, currently has an
Agreement, A-21-194, with Fresno Kings Madera Regional Health Authority dba
CalViva Health, a California Government Entity, which was approved by the Board of
Supervisors on June 8, 2021, to lease 100 square feet of county-owned office space at
1221 Fulton Street to provide health care member services to residents.
Pursuant to Paragraph 2 of Agreement No. A-21-194, this letter will serve as the
County's thirty (30) day advance notice of an intention to terminate the above
Agreement. The effective date of termination shall be June 8, 2022, to have the leased
location vacated and any items or equipment returned to their respective owners.
