Re: One-Year Extension of Agreement#19-444 (Master Agreement for the Provision of Specialty and
Primary Professional Medical Services and Emergency Care)
Dear Ms. Fields-Keene;
Fresno County Agreement#19-444, for the provision of specialty and primary professional medical
services and emergency care was approved by the Fresno County Board of Supervisors on September 10,
2019. The contract includes an option to extend for seven individual consecutive 12-month terms upon
written approval of both parties no later than 60-days prior to the first day of the next 12-month extension
term (September 10, 2022) and authorized the Department of Public Health Director, or designee,to
execute such written approval on behalf of the County.
Contract service performance and need was reviewed by the Department, and it was determined that
continued collaboration is needed. The Department provides this letter as notice of renewal, extending the
Agreement for the second 12-month extension term(September 10, 2022,through September 9, 2023)
available per this agreement beyond the current term. If you, as the signing authority for the Central
California Faculty Medical Group,would also like to extend the agreement as stated,please sign and date
on the line below and return this letter by July 1, 2022,to:
