The County of Fresno, through its Department of Behavioral Health (DBH), has
executed Master Agreement No. 20-015 for providers to use telecommunications and
information technology to provide specialty mental health services across distance
(telemedicine).
Pursuant to Section 9, beginning on page 13, line 8 of Agreement No. 20-015,
"COUNTY's DBH Director, or designee, reserves the right at any time during the term of
this Agreement to add CONTRACTOR(S) to Exhibit A, "List of Contractors".
Pursuant to Section 5, "Compensation" of Agreement No. 20-015, Locumtenens.com
has proposed the rates as per Exhibit B, (copy attached) which shall be effective
immediately.
Please accept this acknowledgment letter as notice that Fresno County is in receipt of
the attached signature page from Dr.Wanted.com, LLC agreeing to the terms and
conditions of Master Agreement No. 20-015 and has been added to the Master
Agreement No. 20-015.
