The County of Fresno, through its Department of Behavioral Health, has executed
Master Agreement No. 19-200 for contractors to provide licensed psychiatrists as
needed to deliver psychiatric services to individuals in Fresno County.
Pursuant to Section 16, beginning on page 19, line 17 of Agreement No. 19-200,
"COUNTY's DBH Director, or designee, reserves the right at any time during the term of
this Agreement to add new Contractors to those contained in Exhibit A."
Pursuant to Section 5, "Compensation" of Agreement No. 19-200 Dr.Wanted.com,
LLC has proposed the rates as per Exhibit B, (copy attached).
Please accept this acknowledgment letter as notice that the County of Fresno is in
receipt of the attached signature page from Dr.Wanted.com, LLC agreeing to the terms
and conditions of Master Agreement No. 19-200 and has been added to the Master
Agreement No. 19-200, effective immediately.

Daily All-Inclusive Rate for Each Psychiatrist: $300-$400/hour

5 8:00 a.m. through 5:00 p.m. Monday through
Sunday, with a 40 hour maximum per week)

Daily All-Inclusive Rate-Forensic Services: $300-$420/hour
7 (This rate is for services provided between
8 8:00am-5:00pm Monday through Sunday
with a 40-hour maximum per week, with an
9 emphasis on services performed in the County
Jail or at trial evaluations.)

11 Weekend Call Rate (This Rate is for carrying: $1200-$1500/1ight
a pager and taking call via phone from Saturday
12 at 5:00 p.m. through Monday at 8:00 a.m., when the
psychiatrist is scheduled less than eight hours
13 on Saturday or Sunday)

In-House Call Rate (This Rate is for carrying: $1,70042,100/night
15 a pager and taking call via phone from Saturday
at 5:00 p.m. through Monday at 8:00 a.m., when the
16 psychiatrist is scheduled to work an eight hour shift on
17 Saturday or Sunday between 8:00 a.m. and 5:00 p.m.)
18 Weeknight Call Rate (This Rate is for carrying: $450-600/night
a pager and taking call via phone from 5:00 p.m.
19 Monday through 8:00 a.m. Saturday, no matter
