RE: Six (6) Month Extension of Agreement for Alternate Care Site Staffing
Dear Mr. Cook:
Agreement#20-418, which was approved on August 7, 2020, including the First Amendment
#20-418-1 approved on December 12, 2020, allowed for the option of a six (6) month extension
upon written approval of both parties no later than thirty days prior to the first day of the next six
(6) month extension period (July 1, 2021). The Director of the Department of Public Health is
authorized to execute such written approval on behalf of the County.
I have reviewed our agreement with AMI Expeditionary Healthcare LLC in providing the
contracted services and based on AMI's satisfactory performance of service we desire to
maintain a "stand-by" relationship for an additional six (6) months in the event your services are
necessary. Therefore, I would like to extend the agreement from July 1, 2021 through
December 31, 2021. If you, as the signing authority for AMI Expeditionary Healthcare LLC
would also like the agreement to be extended as stated, please sign and date on the line below
and return this letter to:
