Make a finding that it is in the best interest of the County to waive the competitive bidding process consistent with Administrative Policy No. 34 for unusual or extraordinary circumstances for the solicitation of services to provide group prenatal care services as part of an integrated prenatal care system in Fresno County; and 2. Approve and authorize the Chairman to execute a sole source Agreement with Children and Families Commission of Fresno County,for the Glow! Group Prenatal Care and Support project, effective upon execution through March 31, 2024,total not to exceed $468,000. There is no additional Net County Cost associated with the recommended actions. Approval of the recommended actions will allow the Department of Public Health (Department)to partner with Children and Families Commission of Fresno County(F5FC)to enhance the services provided through the Glow! Group Prenatal Care and Support(Glow!) project. The project aligns with the goals and activities under the Healthy Start Initiative grant for which the Department receives funding. Collaboration with the F5FC project will maximize resources for the benefit of the community. This partnership will provide group prenatal care services that include curriculum and wrap-around services in partnership with medical provider offices to enhance support for pregnant women. This item is countywide. ALTERNATIVE ACTION(S): There are no viable alternative actions. Should your Board not approve the recommended actions, the Department would not be able to collaborate with F5FC to provide enhanced group prenatal care services and would duplicate efforts to set up a similar program with reduced funding under the County's Federal Healthy Start grant. SUSPENSION OF COMPETITION/SOLE SOURCE CONTRACT: The Department requests to waive the competitive bidding process consistent with Administrative Policy No. 34, as in July 2019, F5FC bid out group prenatal care services to include curriculum and wrap-around services in partnership with medical provider offices. The Department participated in the evaluation team that selected F5FC's current service providers. F5FC is the only agency that has implemented this prenatal care service model in the community, and it would be in the County's best interest to provide funding to F5FC to maximize different funding streams and fulfill the County's required services under the Healthy Start funding. The Internal Services Department- Purchasing concurs with the Department's request to suspend the competitive bidding process. The recommended agreement with FSFC to lead the Glow! project has a consecutive three-year total maximum not to exceed $468,000 and will be funded with the Federal Healthy Start Initiative grant. Sufficient appropriations and estimated revenues are included in the Department Org 5620 FY 21-22 Adopted Budget and will be included in future budget requests for the duration of the contract. DISCUSSION: The Department has a current collaborative partnership with FSFC under various programs that aim to increase positive birth outcomes through education and service linkage. Both the Department and FSFC receive funding from various sources that concentrate in various service models and some specific populations who have historically had poor birth outcomes. Most recently in July 2019, FSFC released a Request for Qualifications (RFQ)to provide group prenatal care services as part of an integrated prenatal care system. The services and goals are aligned with some of the services and goals under the most current Federal Healthy Start Initiative grant activities as implementation of Glow! includes curriculum and wrap-around services, in partnership with medical provider offices, to enhance supports for pregnant women and to demonstrate the effectiveness of group prenatal care in addressing the social determinants of health. Each group or cohort is co-facilitated by a Glow! Staff Facilitator and a Medical Provider Facilitator. The Glow! Staff Facilitator prepares session materials and leads the educational components of each Glow! session, while the Medical Provider Facilitator is responsible for providing the prenatal medical care to patients in each session. The County has received Federal Healthy Start funds, in part, to support the same population group with the same prenatal care services. As a result of FSFC's successful RFQ process and the Department's participation in the selection of service contractors to provide the required group prenatal services,the Department has determined it is in the County's best interest to provide funding to FSFC to fulfill the County's required services. Approval of the recommended actions will allow FSFC and the County to collaborate in a cohesive community approach in providing these services,which will most effectively serve Fresno County residents. The additional funding will enable FSFC to enhance and expand the group prenatal care services being provided by FSFC's existing service providers, as well as fulfill the County's grant funded requirements for the same services. ATTACHMENTS INCLUDED AND/OR ON FILE: On file with Clerk-Agreement with Children and Families Commission of Fresno County Sole Source Acquisition Request CAO ANALYST: Ron Alexander County of Fresno page 2 File Number:22-0277 Agreement No. 22-159 1 AGREEMENT 2 3 THIS AGREEMENT(the "Agreement') is made and entered into this 19th day of April 2022 4 (the "Effective Date"), by and between the COUNTY OF FRESNO, a Political Subdivision of the State of 5 California, hereinafter referred to as "COUNTY", and Children and Families Commission of Fresno County, 6 a California public organization, whose address is 2405 Tulare Street, Suite 200, Fresno, CA, 93721, 7 hereinafter referred to as "CONTRACTOR". 8 WITNESSETH: 9 WHEREAS, COUNTY, through its Department of Public Health (DPH), wishes to support the 10 provision of non-medical care services, including curriculum and wrap-around services, provided as part of 11 the Glow! Group Prenatal Care and Support Program (Glow!), to qualified participants to enhance supports 12 for pregnant women; and 13 WHEREAS, CONTRACTOR, is the only agency that has implemented this prenatal care service 14 model in the community through implementation of Glow!; and 15 WHEREAS, CONTRACTOR, is qualified and willing to provide such services, pursuant to the terms 16 and conditions of this Agreement. 17 NOW, THEREFORE, in consideration of the mutual covenants, terms and conditions herein 18 contained, the parties hereto agree as follows: 19 1. OBLIGATIONS OF THE CONTRACTOR 20 CONTRACTOR agrees to perform the following services (the "Services"): 21 A. Provide the non-medical care services component of the Glow! Program, to 22 qualified participants as set forth in Exhibit A, which is attached hereto and incorporated herein by this 23 reference. In addition to the staffing set forth in Exhibit B, CONTRACTOR shall use Up to 6 Glow! Staff 24 Facilitators to provide services detailed in Exhibit A. The costs of these Staff Facilitators will not be paid 25 by COUNTY under this Agreement but will be paid through a different funding source available to 26 CONTRACTOR. 27 28 29153220 /16453.0001 -1- 1 B. Serve a target number of 200 pregnant women per calendar year (Participants), 2 and subject to Section 13 herein, the target participation percentage of African American women shall 3 be seventy percent (70%). Participants must live in Babies First target area within the City of Fresno. 4 C. Participate in quality improvement (Plan-Do-Study-Act) and evaluation efforts 5 (focus groups, satisfaction surveys, etc.) as needed. 6 D. Participate in monthly calls with COUNTY regarding progress and submit 7 Services update on COUNTY-provided template. 8 E. Participate in quarterly Community Advisory Network (CAN) meetings and provide 9 updates to COUNTY as requested. 10 F. Complete identified trainings and Healthy Start trainings to ensure a quality 11 workforce. 12 G. Glow! Program manager to sign up to receive periodic updates from Healthy 13 Start. 14 H. Collect additional monthly data as mandated by the program funder. 15 I. Complete and update the following Healthy Start required forms: Background 16 Form and Prenatal Form. Forms will be completed at client enrollment and updated after the 17 Participant's child is born. 18 J. Upon Participant's completion of the Glow! Program, CONTRACTOR shall offer 19 eligible Participants Babies First Care Coordination services. 20 K. Submit completed Healthy Start forms to be entered into CAREWare, the 21 database used by Healthy Start grantees within two (2) weeks of enrollment or by the 5th day of each 22 month, whichever is sooner. 23 L. Provide Glow! Program overview to COUNTY, including staffing, role, and 24 position description, and written updates for progress report and annual report, including the Work Plan, 25 attached hereto as Exhibit A. 26 M. Establish a process to identify the father of Participant's child or partners for 27 purposes of connecting such person to the appropriate subcontractor to offer fatherhood services. 28 29153220/16453.0001 -2- 1 Share program information, resources and references with all appropriate parties in order to maximize 2 services to eligible women and families. 3 2. TERM 4 The term of this Agreement shall be effective from the Effective Date through and including March 5 31, 2024, unless sooner terminated as provided herein (the "Term"). 6 3. TERMINATION 7 A. Non-Allocation of Funds—The terms of this Agreement, and the Services to be 8 provided hereunder, are contingent on the approval of funds by the appropriating government agency. 9 Should sufficient funds not be allocated, the services provided may be modified, or this Agreement 10 terminated, at any time by giving the CONTRACTOR thirty (30) days advance written notice. 11 B. Breach of Contract—Either Party may terminate this Agreement, effective upon 12 written notice to the other Party (the"Defaulting Party"), if the Defaulting Party materially breaches this 13 Agreement, and such breach is incapable of cure, or concerning a breach capable of cure, the Defaulting 14 Party does not cure such breach within thirty (30) days after written notice of such breach. The COUNTY 15 may immediately suspend or terminate this Agreement in whole or in part, where in the determination of the 16 COUNTY there is: 17 1) An illegal or improper use of funds; 18 2) A failure to comply with any term of this Agreement; 19 3) A substantially incorrect or incomplete report submitted to the COUNTY; 20 4) Improperly performed service. 21 In no event shall any payment by the COUNTY constitute a waiver by the COUNTY of any breach 22 of this Agreement or any default which may then exist on the part of the CONTRACTOR. Neither shall 23 such payment impair or prejudice any remedy available to the COUNTY with respect to the breach or 24 default. The COUNTY shall have the right to demand of the CONTRACTOR the repayment to the 25 COUNTY of any funds disbursed to the CONTRACTOR under this Agreement, which in the judgement of 26 the COUNTY were not expended in accordance with the terms of this Agreement. The CONTRACTOR 27 shall promptly refund any such funds upon demand. 28 HI 29153220/16453.0001 -3- 1 C. Without Cause—Under circumstances other than those set forth above,this 2 Agreement may be terminated by COUNTY upon the giving of thirty (30) days advance written notice of 3 intention to terminate to CONTRACTOR. 4 4. COMPENSATION/INVOICING 5 COUNTY agrees to pay CONTRACTOR and CONTRACTOR agrees to receive compensation as 6 identified in Exhibit B, attached hereto and incorporated herein by this reference. CONTRACTOR shall 7 submit monthly invoices by the 30th day of each month for the prior month's expenditures to the County of 8 Fresno, Department of Public Health, Public Health Nursing Division, P.O. Box 11867, Fresno, CA 93775, 9 Attention: Division Manager at DPHBOAP@fresnocountyca.gov. Invoices shall detail line items as 10 specified in Exhibit B, including original budget amounts, current month's expenses, year-to-date expenses 11 and budget balances. In addition, invoices shall also include all relevant supporting documentation 12 including but not limited to copies of original statements, general ledgers, program expense receipts, payroll 13 records and mileage claims. 14 In no event shall the total maximum compensation for the full term of the Agreement exceed Four 15 Hundred Sixty-Eight Thousand and No/100 Dollars ($468,000.00). It is understood that all expenses 16 incidental to CONTRACTOR'S performance of services under this Agreement shall be borne by 17 CONTRACTOR. 18 5. FUNDING 19 Funding for these services is supported by the Health Resources and Services Administration 20 (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of an award totaling Nine 21 Hundred Eighty Thousand and No/100 Dollars ($980,000.00)with 0% financed with nongovernmental 22 sources (Catalog of Federal Domestic Assistance Number 93.926). The contents are those of the author(s) 23 and do not necessarily represent the official views of, nor an endorsement, by HRSA, HHS or the Federal 24 government. 25 6. INDEPENDENT CONTRACTOR 26 In performance of the work, duties and obligations assumed by CONTRACTOR under this 27 Agreement, it is mutually understood and agreed that CONTRACTOR, including any and all of the 28 CONTRACTOR'S officers, agents, and employees will at all times be acting and performing as an 29153220/16453.0001 -4- 1 independent contractor, and shall act in an independent capacity and not as an officer, agent, servant, 2 employee,joint venturer, partner, or associate of the COUNTY. Furthermore, COUNTY shall have no right 3 to control or supervise or direct the manner or method by which CONTRACTOR shall perform its work and 4 function. However, COUNTY shall retain the right to administer this Agreement so as to verify that 5 CONTRACTOR is performing its obligations in accordance with the terms and conditions thereof. 6 CONTRACTOR and COUNTY shall comply with all applicable provisions of law and the rules and 7 regulations, if any, of governmental authorities having jurisdiction over matters the subject thereof. 8 Because of its status as an independent contractor, CONTRACTOR shall have absolutely no right 9 to employment rights and benefits available to COUNTY employees. CONTRACTOR shall be solely liable 10 and responsible for providing to, or on behalf of, its employees all legally-required employee benefits. In 11 addition, CONTRACTOR shall be solely responsible and save COUNTY harmless from all matters relating 12 to payment of CONTRACTOR'S employees, including compliance with Social Security withholding and all 13 other regulations governing such matters. It is acknowledged that during the term of this Agreement, 14 CONTRACTOR may be providing services to others unrelated to the COUNTY or to this Agreement. 15 7. MODIFICATION 16 Any matters of this Agreement may be modified from time to time by the written consent of all the 17 parties without, in any way, affecting the remainder. 18 Notwithstanding the above, changes to line items in the budget, attached hereto as Exhibit B, that 19 do not exceed ten percent (10%) of the maximum compensation payable to the CONTRACTOR, may be 20 made with the written approval of COUNTY's DPH Director, or designee. Said budget line item changes 21 shall not result in any change to the maximum compensation amount payable to CONTRACTOR, as stated 22 herein. 23 8. NON-ASSIGNMENT 24 Neither party shall assign, transfer or sub-contract this Agreement nor their rights or duties under 25 this Agreement without the prior written consent of the other party. 26 9. HOLD HARMLESS 27 CONTRACTOR agrees to indemnify, save, hold harmless, and at COUNTY'S request, defend the 28 COUNTY, its officers, agents, and employees from any and all costs and expenses (including attorney's 29153220/16453.0001 -5- 1 fees and costs), damages, liabilities, claims, and losses occurring or resulting to COUNTY in connection 2 with the performance, or failure to perform, by CONTRACTOR, its officers, agents, or employees under this 3 Agreement, and from any and all costs and expenses (including attorney's fees and costs), damages, 4 liabilities, claims, and losses occurring or resulting to any person, firm, or corporation who may be injured 5 or damaged by the performance, or failure to perform, of CONTRACTOR, its officers, agents, or 6 employees under this Agreement. 7 The provisions of this Section 9 shall survive termination of this Agreement. 8 10. INSURANCE 9 Without limiting the COUNTY's right to obtain indemnification from CONTRACTOR or any third 10 parties, CONTRACTOR, at its sole expense, shall maintain in full force and effect, the following 11 insurance policies or a program of self-insurance, including but not limited to, an insurance pooling 12 arrangement or Joint Powers Agreement (JPA) throughout the term of this Agreement: 13 A. Commercial General Liability 14 Commercial General Liability Insurance with limits of not less than Two Million Dollars 15 ($2,000,000.00) per occurrence and an annual aggregate of Four Million Dollars ($4,000,000.00). This 16 policy shall be issued on a per occurrence basis. COUNTY may require specific coverages including 17 completed operations, products liability, contractual liability, Explosion-Collapse-Underground, fire legal 18 liability or any other liability insurance deemed necessary because of the nature of this contract. 19 B. Automobile Liability 20 Comprehensive Automobile Liability Insurance with limits of not less than One Million 21 Dollars ($1,000,000.00) per accident for bodily injury and for property damages. Coverage should include 22 any auto used in connection with this Agreement. 23 C. Professional Liability 24 If CONTRACTOR employs licensed professional staff, (e.g., Ph.D., R.N., L.C.S.W., 25 M.F.C.C.) in providing services, Professional Liability Insurance with limits of not less than One Million 26 Dollars ($1,000,000.00) per occurrence, Three Million Dollars ($3,000,000.00) annual aggregate. If 27 CONTRACTOR employs licensed professional staff, (e.g., Ph.D., R.N., L.C.S.W., M.F.C.C.) in providing 28 services, CONTRACTOR agrees that it shall maintain, at its sole expense, in full force and effect for a 29153220 /16453.0001 -6- 1 period of three (3) years following the termination of this Agreement, one or more policies of professional 2 liability insurance with limits of coverage as specified herein. 3 D. Worker's Compensation 4 A policy of Worker's Compensation insurance as may be required by the California Labor 5 Code. 6 E. Molestation 7 Sexual abuse/molestation liability insurance with limits of not less than One Million Dollars 8 ($1,000,000.00) per occurrence. Two Million Dollars ($2,000,000.00) annual aggregate. This policy shall 9 be issued on a per occurrence basis. 10 F. Cyber Liability 11 Cyber Liability Insurance, with limits not less than Two Million Dollars ($2,000,000) per 12 occurrence or claim, Two Million Dollars ($2,000,000) annual aggregate. Coverage shall be sufficiently 13 broad to respond to the duties and obligations as is undertaken by the CONTRACTOR in this Agreement 14 and shall include, but not be limited to, claims involving infringement of intellectual property, including but 15 not limited to infringement of copyright, trademark, trade dress, invasion of privacy violations, information 16 theft, damage to or destruction of electronic information, release of private information, alteration of 17 electronic information, extortion and network security. The policy shall provide coverage for breach 18 response costs as well as regulatory fines and penalties as well as credit monitoring expenses with limits 19 sufficient to respond to these obligations. 20 For purposes of the cyber liability insurance required under this Agreement, "cyber risks" 21 include, but are not limited to, (i) security breaches, which include disclosure of, whether intentional or 22 unintentional, information provided by the COUNTY or Confidential Information; (ii) breach of any of the 23 CONTRACTOR's obligations under this Agreement relating to data security, protection, preservation, 24 usage, storage, transmission, and the like; (iii) infringement of intellectual property including, but not limited 25 to, infringement of copyright, trademark, and trade dress; (iv) invasion of privacy, including any release of 26 private information; (v) information theft by any person or entity, whatsoever; (vi) damage to, destruction of, 27 or alteration of electronic information; (vii) extortion related to the CONTRACTOR's obligations under this 28 Agreement regarding electronic information, including information provided by the COUNTY, information 29153220 /'16453.0001 -7- 1 provided by or obtained from any person examined or person questioned, Confidential Information, or 2 personal-identifying information relating to any person; (viii) network security; (ix) data breach response 3 costs, including security breach response costs; (x) regulatory fines and penalties related to the 4 CONTRACTOR's obligations under this Agreement regarding electronic information, including information 5 provided by the COUNTY, information provided by or obtained from an examinee or person questioned, or 6 personal-identifying information relating to any examinee or person questioned; and (xi) credit monitoring 7 expenses. 8 Additional Requirements Relating to Insurance 9 CONTRACTOR shall obtain endorsements to the Commercial General Liability insurance naming 10 the COUNTY, its officers, agents, and employees, individually and collectively, as additional insured, but 11 only insofar as the operations under this Agreement are concerned. Such coverage for additional insured 12 shall apply as primary insurance and any other insurance, or self-insurance, maintained by COUNTY, its 13 officers, agents and employees shall be excess only and not contributing with insurance provided under 14 CONTRACTOR's policies herein. This insurance shall not be cancelled or changed without a minimum of 15 thirty (30) days advance written notice given to COUNTY. 16 Within thirty (30) days from the date CONTRACTOR signs and executes this Agreement, 17 CONTRACTOR shall provide certificates of insurance and endorsement as stated above for all of the 18 foregoing policies, as required herein, to the County of Fresno, Department of Public Health, P.O. Box 19 11867, Fresno, CA 93775, Attention: Contracts Section —6th Floor, stating that such insurance coverage 20 have been obtained and are in full force; that the COUNTY, its officers, agents and employees will not be 21 responsible for any premiums on the policies; that such Commercial General Liability insurance names the 22 COUNTY, its officers, agents and employees, individually and collectively, as additional insured, but only 23 insofar as the operations under this Agreement are concerned; that such coverage for additional insured 24 shall apply as primary insurance and any other insurance, or self-insurance, maintained by COUNTY, its 25 officers, agents and employees, shall be excess only and not contributing with insurance provided under 26 CONTRACTOR's policies herein; and that this insurance shall not be cancelled or changed without a 27 minimum of thirty (30) days advance, written notice given to COUNTY. 28 In the event CONTRACTOR fails to keep in effect at all times insurance coverage as herein 29153220 /16453.0001 -8- 1 provided, the COUNTY may, in addition to other remedies it may have, suspend or terminate this 2 Agreement upon the occurrence of such event. 3 All policies shall be issued by admitted insurers licensed to do business in the State of California, 4 and such insurance shall be purchased from companies possessing a current A.M. Best, Inc. rating of A 5 FSC VI or better. 6 11. AUDITS AND INSPECTIONS 7 The CONTRACTOR shall at any time during business hours, and as often as the COUNTY may 8 deem necessary, make available to the COUNTY for examination all of its records and data with respect to 9 the matters covered by this Agreement. The CONTRACTOR shall, upon request by the COUNTY, permit 10 the COUNTY to audit and inspect all of such records and data necessary to ensure CONTRACTOR'S 11 compliance with the terms of this Agreement. 12 If this Agreement exceeds Ten Thousand and No/100 Dollars ($10,000.00), CONTRACTOR shall 13 be subject to the examination and audit of the Auditor General for a period of three (3) years after final 14 payment under contract (Government Code Section 8546.7). 15 12. CONFIDENTIALITY 16 All services performed by CONTRACTOR under this Agreement shall be in strict conformance with 17 all applicable Federal, State of California and/or local laws and regulations relating to confidentiality. 18 13. NON-DISCRIMINATION 19 During the performance of this Agreement, CONTRACTOR shall not unlawfully discriminate against 20 any employee or applicant for employment, or recipient of services, because of race, religious creed, color, 21 national origin, ancestry, physical disability, mental disability, medical condition, genetic information, marital 22 status, sex, gender, gender identity, gender expression, age, sexual orientation, military status or veteran 23 status pursuant to all applicable State of California and Federal statutes and regulation. 24 14. COMPLIANCE WITH FEDERAL REQUIREMENTS 25 CONTRACTOR recognizes that COUNTY operates its DPH Babies First program under an 26 agreement with the HRSA, HHS, Maternal and Child Health Bureau, Grant No. H49MC00150, incorporated 27 herein by reference; and that under said agreement the Federal government imposes certain requirements 28 on COUNTY and its subcontractors. Therefore, CONTRACTOR shall adhere to the Federal requirements 29153220/16453.0001 -9- 1 identified in said agreement. COUNTY shall make available to CONTRACTOR a copy of said agreement 2 upon request. 3 15. SINGLE AUDIT CLAUSE 4 A. If CONTRACTOR expends Seven Hundred Fifty Thousand and No/100 Dollars 5 ($750,000) or more of Federal and Federal flow-through monies, CONTRACTOR agrees to conduct an 6 annual audit in accordance with the requirements of the Single Audit Standards as set forth in Office of 7 Management and Budget (OMB) Title 2 of the Code of Federal Regulations, Chapter II, Part 200. 8 CONTRACTOR shall submit said audit and management letter to COUNTY. The audit must include a 9 statement of findings or a statement that there were no findings. If there were negative findings, 10 CONTRACTOR must include a corrective action plan signed by an authorized individual. CONTRACTOR 11 agrees to take action to correct any material non-compliance or weakness found as a result of such audit. 12 Such audit shall be delivered to COUNTY'S DPH Administration for review within nine (9) months of the 13 end of any fiscal year in which funds were expended and/or received for the program. Work Plan Overview First 5 Fresno County provides group prenatal care in partnership with various clinic practices as part of the EMBRACE Study throughout Fresno County. The Glow! Group Prenatal Care model includes prenatal medical care, risk assessments, group support and information and resources related to pregnancy, birth and parenting. Each group or cohort is co-facilitated by a Glow! Staff Facilitator(First 5 Contractor Network Staff) and a Provider Facilitator(Midwife, Nurse Practitioner, Physician or Physician's Assistant). The role of the Glow! Staff Facilitator is to prepare session materials and lead the educational components of each Glow! session. While the Provider Facilitator is responsible for providing the prenatal medical care to patients in each session. Introduction This work plan provides an overview of the Glow! Group Prenatal Care and Support Program (Glow!) telehealth and in-person models. As COVID-19 transmission rates continue to drop, Glow! will work on transitioning back to an in-person model. However, we anticipate that the program will look different based on the needs of each clinic site. For more detailed program information, please reference the Glow! Program Protocol. What is Glow! Glow! is an innovative and comprehensive program that incorporates wrap-around services with enhanced prenatal care. It allows women to have more time with their medical provider and encourages them to stay actively engaged in their healthcare. The program is rooted in a deep sense of community where women can feel supported, learn from one another while learning from their provider and receive respectful prenatal care in a facilitative manner. Comprehensive wrap-around services are provided to each participant at each session and are designed to ease their access to prenatal care and resources without the burden of having to go to different locations. These services include providing in-home early learning activities for children ages 0 to 5 while the participant joins the Glow! session (telehealth model), childcare for children ages 0-5 (in-person model), transportation stipends, take-home food support, information to community resources through presentations and ongoing assistance with resources not available through the Glow! Sessions 1-4 are one month apart, sessions 5-10 are two weeks apart, and the post- partum session is one month after session 10. Postpartum sessions are important to ensure participants receive this follow-up care which is often missed as infant appointments are prioritized. This is an additional opportunity to provide perinatal wellness referrals and lactation consultation as well as contraception care/family planning. Sessions begin and end with clinical care,this involves participants taking many of their own vital signs and a brief individual physical exam with their provider.This aspect of group prenatal care differs from individual or traditional prenatal care and participants report feeling empowered and having a since of ownership over their health through this activity. After the initial clinical care ends, participants then come together for an opening activity to remind everyone that they are both welcome and an important part of the Centering community and brings the group into cohesion before moving into the facilitated group discussion.The facilitated group session includes a mindfulness exercise, group discussion and activities and ends with a closing. If a wrap-around community resource presentation is scheduled,this is included in the group discussion portion of the session.The final time in the session is the remainder of clinical care for the participants that have not been seen by their provider. The Glow! Staff Facilitator is responsible for coordinating all session aspects including leading the group session, preparing for the session, attending required trainings, working closely with the healthcare team who is delivering prenatal care, communicate with participants, and coordinating wrap-around services/community resource presentations. In July 2019, First 5 Fresno County released a Request for Qualifications (RFQ) to provide group prenatal care services for First 5's implementation of an integrated prenatal care system. Implementation of the Glow! Group Prenatal Care and Support Project (Glow!/GC) includes curriculum and wrap-around services, in partnership with medical provider offices, to enhance supports for pregnant women and to demonstrate the effectiveness of group prenatal care in addressing the social determinants of health. The County of Fresno also requires the same services to be provided, as a condition of its Federal Healthy Start grant funding. As a result of First 5's successful RFQ process and selection of service contractors to provide the required group prenatal services, the Department of Public Health has determined it is in the County's best interest to provide funding to First 5 Fresno County to fulfill the County's required services. The additional funding will enable First 5 Fresno County to enhance the group prenatal care services already being provided by First 5's existing service providers, as well as fulfill the County's grant funded requirements for the same services. 2. In July 2019, First 5 Fresno County bid out group prenatal care services to include curriculum and wrap- around services, in partnership with medical provider offices. The County of Fresno Department of Public Health participated in the evaluation team that selected First 5's current service provider. First 5 is the only agency that has implemented this prenatal care service model in the community. Due to First 5's newly established implementation of this curriculum and service model, and the Department's participation in the selection of First 5's vendor, the Department has determined it to be in the community's best interest to provide additional funding to First 5 to enhance existing services through their current provider. This will allow First 5 and the County to collaborate in a cohesive community approach in providing these services, which will most effectively serve Fresno County residents. 5. E-PD-047 (02-2013) In July 2019, First 5 Fresno County released a bid for quotation to local agencies to provide enhanced group prenatal care services, in conjunction with medical services, to low-income and non-white women. First 5 received three (3) quotations to provide services in the targeted service areas, specifically in areas servicing African American families. All three agencies were funded at varying levels. Therefore, rather than issue a new RFP for the same services, the Department has determined that it would be in the County's best interest to partner with First 5 to provide funding supporting and enhancing services already contracted for through First 5.
