RE: One-Year Extension of Agreement No. A-19-283
Dear Ms. Tupper:
Agreement No. 19-283, which was approved by the Board of Supervisors on June 18,
2019, allows for the option of two one-year extensions upon written approval of both
parties no later than thirty days prior to the first day of the next twelve-month extensive
period, and authorized the Director to execute such written approval on behalf of the
County.
Department staff have reviewed the performance of Kaweah Delta Mental Health
Hospital in providing the contracted services and find it continues to be satisfactory.
Therefore, I would like to extend the agreement from July 1, 2022 through June 30,
2023. If you, as the signing authority for Kaweah Delta Mental Health Hospital, would
also like the agreement to be extended as stated, please sign and date on the line
below and return this letter by e-mailing a copy of the signed letter to Maya Solano-
McDaniel, Staff Analyst at msolanomcdaniel(o-)fresnocountyca.gov.
