Re: Agreement No. 21-536, Evaluation Services for the Advancing Health Literacy Grant
Program
Agreement No. 21-536, which was approved by the Fresno County Board of Supervisors on
December 14, 2021, includes a modification provision that allows modifications to the budget
line items by the written consent of all the parties so long as the modification does not exceed
10% of the maximum compensation and does not change the remainder of the contract terms.
The Director of the Department of Public Health is authorized to execute such written approval
on behalf of the County.
The attached budget modification (1.3.22 Mod. CVHPI Fresno Co Health Literacy Eval Budget)
was requested from the Advancing Health Literacy Grant Program evaluator, California State
University, Fresno Foundation through its Central Valley Health Policy Institute, for the current
agreement term (December 14, 2021 through June 30, 2023), to add a Research Associate
position without any increase to the annual compensation amounts. This modification is less
than 10% of the maximum compensation ($87,500), as allowed in the Agreement.
The Department provides this letter as a notice of modification to Agreement No. 21-536,
pursuant to Section 8 (Modification). If you as the signing authority (or authorized designee) for
California State University, Fresno Foundation agree to the budget modification as stated,
please sign and date on the line below and return this letter to Michael Flores, Staff Analyst at
miflores@fresnocountyca.gov, on or before February 4, 2022.
