Right Perspective. Right Now. December 22, 2021 Gary Cornuelle, Purchasing Manager County of Fresno 2220 Tulare Street, 16th Floor Fresno, CA 93721 Re: County of Fresno Contract No. P-17-499-P Training Course Price Increase Request Dear Gary, When Forensic Analytical Consulting Services, Inc. (dba Hazard Management Services, Inc.) increased our rates with the County of Fresno in 2020, our training fees were inadvertently not included. This request is needed to reflect the increased costs of doing business since our original contract was written in 2017. That said, I’m requesting that we use our rates for training as they were in 2020 (the year of our last price increase with County of Fresno) and not as they stand today. Such an approach seems like a reasonable compromise that serves our mutual interests. Everything is summarized in the following table: TRAINING COST BREAKDOWN DESCRIPTION 2017 2020 2022 2-Hour AHERA Asbestos General Awareness Training $450 $600 $900 16-Hour AHERA Asbestos O&M Initial Training N/A $4,800 $5,100 4-Hour AHERA Asbestos O&M Refresher Training N/A $1,200 $1,500 2-Hour OSHA HAZCOM Lead Worker Training $450 $600 $900 Travel Expenses Central Valley to Central Valley (Per Day) N/A $100 $150 Travel Expenses Bay Area to Central Valley (Per Day) N/A $300 $350 Please accept this request in the spirt of cooperation in which it is intended. A lot has changed for us since 2017. For example, Hazard Management Services, Inc. was acquired by Forensic Analytical Consulting Services, Inc. in 2018. Finally, please remember, I’m only asking to increase our training rates to what they were in 2020 and not to what they will be in 2022. Thank you again for your time and consideration. If you should have any questions, please do not hesitate to call me directly at (303) 945-5792. Respectfully, Forensic Analytical Consulting Services, Inc. David B.
