We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if
this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be
noted above, and perform all services necessary for the work above specified, and will accept as full payment
therefor the prices shown above.
If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the
specifications as to proceeding with the ordered work and filing a written protest within the time therein
specified.
TO: EMMETT’S EXCAVATION, INC:
Make the following changes to the plans and specifications or do the following work not included in the plans
and specifications for this contract. Because of the change(s), TWO WORKING DAYS will be added to the
contract time. The revised date of completion is June 23, 2021.
PART I: DEMO ADDITIONAL CONCRETE IN THE DECK, INVERT, AND WALL SLABS OF THE
GRANT CANAL BOX CULVERT
Drill to a 9-inch depth to bond to the longitudinal (in the direction of the canal) reinforcement with an
approved chemical adhesive to match the existing longitudinal reinforcement within the culvert invert
and wall slabs.
The deck slab reinforcement shall be installed using one of the following options:
Option 1 – Remove the existing concrete to a clean saw cut line to allow the lap of undamaged
existing reinforcing with new longitudinal reinforcement according to Standard Plan D82, Detail A.
Perform lap splicing (minimum length) per Section 52-6.03B, “Lap Splicing,” of the Standard
Specifications.
Option 2 – Remove an additional 6 to 8 inches of concrete to a clean saw cut line exposing between
12 and20 inches of undamaged existing reinforcement. Drill to a 9-inch depth and bond the
longitudinal reinforcement with an approved chemical adhesive to match the existing longitudinal
reinforcement. Provide dowel bars of sufficient length for lap splices according to the requirements of
Section 52-6.03B, “Lap Splicing,” of the Standard Specifications.
All new transverse reinforcement shall be lap splice d with existing transverse reinforcement according
to Section 52-6.03B, “Lap Splicing,” of the Standard Specifications.

TOTAL TIME EXTENSION PART I: ZERO WORKING DAYS
PART II: GRANT A TIME EXTENSION
Grant a time extension of two working days for additional holidays granted by the Department to
contractors currently working according to Section 8-1.07B, “Time Adjustments,” of the Special
Provisions.
