We, the above signed contractor, have given careful consideration to the change proposed and hereby agree, if
this proposal is approved, that we will provide all equipment, furnish all materials, except as may otherwise be
noted above, and perform all services necessary for the work above specified, and will accept as full payment
therefor the prices shown above.
If the Contractor does not sign acceptance of this order, his attention is directed to the requirements of the
specifications as to proceeding with the ordered work and filing a written protest within the time therein
specified.
TO: AVISON CONSTRUCTION, INC.:
Make the following change(s) to the plans and specifications or do work not in the plans and
specifications for this contract. Because of the c hange(s), 136 WORKING DAYS will be added to the
contract time for the First Order of Work.
PART I - GRANT A TIME EXTENSION TO THE FIRST ORDER OF WORK
According to Section 8-1.07B, “Time Adjustments,” of the Special Provisions, grant a 136-working days’
time extension to the allotted contract time for the First Order of Work, Sub mittals. This time extension
is being granted due to the delays caused by repairs to the Garfield Irrigation facilities (work performed
by the Garfield Irrigations District’s contractor), PG&E responses and coordination , and additional time
required by the County of Fresno, the City of Fresno, and the Design Consultants to review and provide
comments regarding material submittals submi tted by the Contractor.
NET COST PART I: NO COST
NET TIME EXTENSION PART I: 136 WORKING DAYS (FIRST ORDER OF WORK)
PART II – BACKFILL REINFORCED CONCRETE PIPE CULVERTS WITH CEMENT SLURRY - EXTRA
WORK
In accordance with Section 4-1.05, “Changes and Extra Work,” of the Standard Specifications, place a
2-sack slurry over two 12 - inch reinforced concrete pipes (RCP) placed across Auberry Road, just
north of Copper Avenue (Approximately sta. 0+40 Auberry Road). These RCP pipes were installed
shallower than the proposed depth due to the alignment conflict with the underground utilities. The
cement slurry material and placement shall conform to the requirements of Section 19-3, Structural
Excavation and Backfill,” of the Standard Specification. The cement slurry backfill shall extend to the
bottom of the roadway section.
Compensation for this work is being made at force account on time and materials basis, as per Section
9-1.03, “Payment Scope,” of the Special Provisions. Payment for this work was calculated according to
Section 9-1.04, “Force Account,” of the Standard Specifications, resulted in $3,534.49.
This work did not impact the critical path for the project, therefore, granting additional contract time was
not warranted.

NET TIME EXTENSION PART II: ZERO WORKING DAYS
PART III – GRADE A DRAINAGE SWALE AT THE NORTHWEST CORNER OF AUBERRY ROAD AND
COPPER AVENUE INTERSECTION - EXTRA WORK
In accordance with Section 4-1.05, “Changes and Extra Work,” of the Standard Specifications, grade a
drainage swale, approximately 6 - inch to 1.0 foot deep, and approximately 2.0 wide, at the northwest
corner, between Sta. 1+50 (Auberry Road) and Sta. 19+15 (Copper Avenue), as directed by the
directed by the Engineer. Perform all work according to Section 19, “Earthwork,” of the Standard
Specifications.
Compensation for this work is being made at force account on time and materials basis, as per Section
9-1.03, “Payment Scope,” of the Special Provisions. Payment for this work was calculated according to
Section 9-1.04, “Force Account,” of the Standard Specifications, resulted in $952.78.
This work did not impact the critical path for the project, therefore, granting additional contract time was
not warranted

NET TIME EXTENSION PART III: ZERO WORKING DAYS
PART IV – REMOVE AND REPLACE TWO ASPHALT DRIVEWAY APPROACHES AND PLACE
HOT MIX ASPHALT ALONG THE RADIUS AT THE NORHTWEST CORNER TO
ADDRESS DRAINAGE CONCERNS – EXTRA WORK
In accordance with Section 4-1.05, “Changes and Extra Work,” of the Standard Specifications, remove
and replace the two newly constructed asphalt driveway approaches; place hot mix asphalt along the
radius; and perform necessary grading to address potential drainage issues at the northwest corner, as
directed by the Engineer. Perform all work according to the applicable sections of the Special
Provisions and Standard Specifications.
Compensation for this work is being made at force account on time and materials basis, as per Section
9-1.03, “Payment Scope,” of the Special Provisions. Payment for this work was calculated according to
Section 9-1.04, “Force Account,” of the Standard Specifications, resulted in $7,660.70.
This work did not impact the critical path for the project, therefore, granting additional contract time was
not warranted.

PART V – GRADE A DRAINGE SWALE ALONG THE SOUTHSIDE OF COPPER AVENUE AND
PLACE A CEMET SLURRY PLUG AROUND THE TRAFFIC LOOP CONDUIT - EXTRA
WORK
In accordance with Section 4 -1.05, “Changes and Extra Work,” of the Standard Specifications, grade a
drainage swale along the southside of Copper Avenue, approximately 1 - foot deep, 2 - feet wide,
between approximate Sta. 16+50 and Sta. 18+00; and construct a slurry seal plug around the traffic
loop conduit on the southside of Copper Avenue, at Sta. 17+25. Perform all work according to the
applicable sections of the Special Provisions and Standard Specifications, as directed by the Engineer.
Compensation for this work is being made at force account on time and materials basis, as per Section
9-1.03, “Payment Scope,” of the Special Provisions. Payment for this work was calculated according to
Section 9-1.04, “Force Account,” of the Standard Specifications, resulted in $3,239.34.
This work did not impact the critical path for the project, therefore, granting additional contract time was
not warranted.
