Po ntiac Way, Clovis, California, 93612, ("LICENSOR"). W I T N E S S E T H: WHEREAS, LICENSOR, through its Department of Behavioral Health (DBH), receives funding from Federal Financial Participation (FFP) Medi-Cal revenue, and Mental Health Services Act (MHSA) Prevention and Early Intervention (PEI) funds; and WHEREAS, there is a significant population of individuals in the Fresno Me tro area of the county who are actively experiencing a mental health crisis and are subject to the negative consequences of inadequate access to appropriate crisis intervention services; and WHEREAS, LICENSEE is willing and able to provide Crisis Intervention Team (CIT) services to individuals experiencing an acute mental health crisis in the Fresno Metro area utilizing a co-response model with law enforcement agencies, pursuant to Fresno County Agreement # 18-688; and WHEREAS, LICENSOR owns the building located at 1925 East Dakota Avenue, Fresno, CA 93726 (the "Building"); and WHEREAS, LICENSOR and LICENSEE mutually desire for LICENSEE to co-locate in the Building in order to provide mental health CIT services. NOW, THEREFORE, in consideration of the mutual promises, covenants and conditions hereinafter contained, such parties, and each of them, agree as follows: 1.LICENSED PREMISES -LICENSOR currently owns and shall make available to LICENSEE approximately 1,600 square feet of office space, at the location commonly known as 1925 East Dakota Avenue, Fresno, CA 93726, as shown in Exhibit "A", which is attached hereto and by this reference incorporated herein (hereinafter "Premises"). 2.GRANT OF LICENSE -LICENSOR grants to LICENSEE a License to use 1 Agreement # D-21-560
