RE: One-Year Extension of Agreement No. 19-200
Dear Karen Mote:
County of Fresno Agreement No. 19-200, which was approved by the Board of Supervisors on May 7, 2019 allowed for
the option of two (2) one-year extensions upon written approval of both parties no later than thirty days prior to the first
day of the next twelve-month extension period, July, 1, 2021, and authorized the Director of the Department of Behavioral
Health to execute such written approval on behalf of the County.
I have reviewed the performance of Cross Country Locums in providing contracted Services and find it continues to be
satisfactory. Therefore, I would like to extend the agreement from July 1, 2021 through June 30, 2022. If you, as the
signing authority for Cross Country Locums, would also like the agreement to be extended as stated, please sign a nd date
on the line below and return this letter by June 1, 2021 to:
