department.
Add the Department of Children & Family Services, contact individual will be Rebecca Vue at
253-9029. The invoice address is 201 1 Fresno Street, Suite 301-Center Court all-3' Floor,
Fresno, CA 93721.
This amendment will be effective immediately through contract period. Prices will be firm for the
contract period. Amendment provisions shall remain in full force and effect with any contract
renewals.
Please acknowledge your acceptance by returning all pages of the signed original of this letter
to my office, retaining a copy for your files.
