Community partners will drive place based anti racism work that will lead to reductions in African American preterm birth and infant mortality in census tracts 2, 7. 9.02, and 10 in the 93706 Zip Code, representing approximately 2700 African American residents. A Fresno GROWS Hub for Civic Engagement is being established at the West Fresno Family Resource Center, which will serve as the backbone organization and provide key administrative and organizational support for the Hub in close partnership with the Fresno Department of Public Health, as required by the City Match. The Backbone is charged with convening community partners, and providing grants oversight and management, as well as monitoring and evaluating progress, and documenting achievements to be disseminated to a wide range of stakeholders. The Fresno GROWS Hub for Civic Engagement will be led by a Steering Committee comprised of Community Champions, community based organizations, academic institutions, and other partners. A results based accountability framework will be applied to craft a monitoring and evaluation plan that tracks agreed upon population level outcomes for both short and long term indicators (e.g., preterm birth, infant mortality), and that tracks programmatic achievements by monitoring performance measures (e.g., number of clients served, fellows hired, % satisfied with services, etc.). Community identified priorities will be addressed through Fresno GROWS driven strategies and by mutually reinforcing activities that are not driven (or funded) by Fresno GROWS, per se, but through the efforts of Fresno partners advancing aligned work that touch Zone residents on the path toward improved birth outcomes in African American families. This workplan will be organized according to key CI domains. These include the Backbone for the Zone, the Common Agenda working groups represented by the 4 pillars:Communications,Collaborations, Capacity building, and Community organizing;Continuous Communication,Shared Measurement, and Community Engagement.Sample measures and timeline for the different activities are provided.
