Daily All-Inclusive Rate
(This Rate is for a schedule of services
provided between 8:00 a.m. through
5:00 p.m. Monday through Sunday,
with a maximum schedule of 40 hours
of service per week)
Weekend Call Rate
(This rate is for the provider being
scheduled for services from Saturday
at 5:00 p.m. through Monday at 8:00
a.m., when the provider is scheduled
for less than eight hours of services on
Saturday or Sunday)
In-House Call Rate
(This rate is for the provider being
scheduled for services from Saturday
at 5:00 p.m. through Monday at 8:00
a.m., when the provider is scheduled
for eight hours of services on Saturday
or Sunday between 8:00 a.m. and 5:00
p.m.)
Weeknight Call Rate
(This rate is for the provider being
scheduled for services from 5:00 p.m.
Monday through 8:00 a.m. Saturday,
no matter whether the provider is
