The County of Fresno (County)hereby contracts with Fresno Survivors of Suicide Loss,
Inc.(Contractor)to provide suicide prevention/education training Services in
accordance with Attachment "A",attached,and by this reference made a part hereof.
TERM:This Agreement shall become effective June 1,2005 and shall remain in effect
through May 31,2006.
RENEWAL:This Agreement shall automatically renew .for an additional one (1)year
period except when written notice is delivered by either party to the other expressing
intent not to renew.Such notice must be delivered a minimum of sixty (60)days prior
to the next expiration date of this Agreement.The maximum number of automatic one
(1)year renewal periods under this Agreement is two (2).
CONTRACTOR'S SERVICES:Contractor shall perform the services as described in
Attachment "A"attached,at the rates set forth in Attachment "A".
ADDITIONAL ITEMS:The County reserves the right to negotiate additional items to
this Agreement as deemed necessary.Such additions shall be made in writing and
signed by both parties.
DEFAULT:In case of default by Contractor,the County may procure the articles from
another source and may recover the loss occasioned thereby from any unpaid balance
due the Contractor or by any other legal means available to the County.The prices
paid by County shall be considered the prevailing market price at the time such
purchase is made.Inspection of deliveries or offers for delivery which do not meet
specifications will be at the expense of Contractor.

Training Services and Compensation
Services
The Contractor shall provide various training workshops on suicide prevention/education
to County's Department of Behavioral Health (DBH)staff.Training topics shall include,
but are not limited to:
.Suicide myths and facts
.Suicidal warning signs
.Referral/resource services

.Use of the national suicide prevention network
Discussing suicide with at-risk individuals.
.Role play
All training will be conducted on County premises.
TraininQ Dates and Times:
All training shall be done at times and dates mutually agreed upon by County's DBH and
Contractor.
Compensation
The Contractor shall be paid One Hundred Twenty-Five and No/100 Dollars ($125)for
each hour of instruction and Five and No/100 Dollars ($5)for each instructional handout.
Each training session will be a minimum of two (2)hours,not to exceed a maximum three
(3)hours.
The maximum amount of compensation under this Agreement shall not exceed Seven-
Thousand Five Hundred and No/100 Dollars ($7,500)for each one (1)year period of this
Agreement.
